Community Pain Management
Chronic pain affects up to 28 million adults in the UK, with 37% of women and 31% of men reporting chronic/persistent pain.
Lincolnshire’s Community Pain Management Service is provided by Connect Health, a leading specialised independent provider of integrated community health services.
It offers access to a range of appointments across multiple sites, delivered by a specialist team including physiotherapists, psychologists, pharmacists & occupational therapists.
Patients will also have access to various resources to support and sustain self-help, empowering patients to understand and manage their pain in the best way possible.
The service also integrates with other community physical and mental health providers including voluntary sector groups.
